First try a new different disk card, my case is the card is not working properly with openwrt along RPI4
If you are using Windows OS,
run cmd with admin, run following command:
diskpart
list disk
(find the disk you are doing, double check, warning:you might lose all your data if you make a mistake!)
select disk 1 (my disk is disk 1, yours, check it, double check it)
clean
All done
